Heel pain is a common presenting complaint to many podiatry clinics, with a wide differential diagnosis including plantar fasciitis, calcaneal stress fracture, Severs Heel (apophysitis) and inflammatory arthritis. One of the conditions that often gets missed or incorrectly diagnosed with relation to heel pain is a condition called baxters heel. This is when the first branch of the lateral plantar nerve gets impinged. This nerve gives sensory feedback but also gives a small muscle (abductor digiti minimi) in the foot its innervation. We usually start with conservative treatment options such as taping, stretching protocols and possible medication to reduce inflammation. We may follow this up with custom orthotics, night splints and mobilisation, The majority of patients will respond to this but in the case of Baxters nerve we may need to inject corticosteroid/local anaesthetic mixture in and around the nerve.
